Sucia Oaks Adoption Contract

Please read the following in full and sign the bottom in acknowledgement &
Agreement of all terms. If for any reason the puppy/dog cannot remain with you, it will be returned to SUCIA OAKS Rescue IMMEDIATELY, without hesitation.

1. If you move, you will notify Sucia Oaks Rescue of your new address &
phone number at least 2 weeks prior to move, so that we may keep in touch
with you.

2. Health Program: The dog adopted shall receive it's Rabies vaccines as
required by law. The dog shall also have yearly booster shots of DHPP-C
and Bordetella. The adopted dog will be seen by a veterinarian within five (5) days of the adoption date for a health check up to ensure proper health. Rescue reserves the right to check with the veterinarian at anytime to verify that all the basic health requirements are being met. Renewal of Rabies vaccine is required by law and therefore by the terms of this contract.
You understand we may ask for proof to verify that these things are being
provided.

3. You will contact us immediately if the puppy/dog is missing (ran away or
taken).

4. You will provide adequate food and water for this puppy/dog and you
agree that this puppy/dog will be kept as a pet.

5. Adequate Fencing
The adopter agrees that the definition of an adequate fence is a fence that encloses a yard or a large pen. An adequate fence is one that prevents the dog within to go out of the area by jumping, digging, or exiting through gates not properly secured. If there is no fence, the dog will not be allowed to roam free without a leash.

6. You will not abuse or neglect the puppy/dog nor will you or any member of the household tie up or chain the puppy/dog outdoors in any manner. You will not allow the puppy/dog to ride in the back of a pick-up truck, unless the truck has an enclosed top

7. You will agree to a post-adoption follow-up and understand that if any
of these conditions are not being met, the puppy/dog may be taken back
IMMEDIATELY.

8. Transfer of Ownership

The dog shall not be sold, given to, or adopted to anyone else, nor shall there be any transfer of ownership to any firm, corporation, or organization of other homeless animals, such as another rescue facility, or research facility.
The dog adopted from rescue must be returned to rescue if for any reason
the adopter can no longer keep it. Violation of this agreement will result in
court costs and attorney fees payable by the adopter if necessary for the rescue to redeem the dog.

You and have been truthful on all the information provided on your application and this contract.

You understand that we do not always have a case history of the puppy/dog
and therefore we do not know every personality quirk or likes and dislikes and that all traits might not be exhibited to us during our evaluation period

You understand and agree Sucia Oaks Rescue will not be held responsible for
any damages or injuries by this puppy/dog and that Sucia Oaks Rescue is not
always aware of preexisting conditions due to the animal being a rescue and will not be held responsible for the same.

You understand and acknowledge it is Sucia Oaks Rescue's purpose to find these animals permanent, loving families

You WILL call us day or night if you have ANY questions or concerns
regarding any aspect of this adopted animal.
